Article X. IDENTITY THEFT PREVENTION PROGRAM |
§ 11-204. Process of establishing a covered account.
(a)
As a precondition to opening a covered account in the city, each applicant shall provide the city with personal identifying information of the customer:
(1)
A valid government issued identification card containing a photograph of the customer.
(2)
Social Security Number.
(3)
If a non U.S. Citizen, a valid photo identification card and appropriate issued visa, passport and/or resident card.
(4)
Such applicant shall also provide any information necessary for the department providing the service for which the covered account is created to access the applicant's consumer credit report.
